Essential Job Function:
Collaborate closely with hospital discharge planners, social workers, case managers, physicians and management, as well as provide education staff to generate referrals.
Develop and retain strong relationships with key referral sources while identifying opportunities for partnering and growth.
Manage leads from referral sources and conduct intake assessments on these All of this information input into salesforce for tracking purposes.
Once a referral has been identified, and assessed, the resource coordinator will submit referral packet to insurance, for authorization.
Coordinate with clinical assessment team on referrals for admission based on the services provided by the facility.
Reviewing packets from referral sources, for patient(s) placement into Millcreek.
Educate patients, families, community and other referral sources on programs, services and amenities provided within facility.
Coordinate and complete assessments with potential patients.
Submitting these internally approved patients to their PASSE, or insurance provider for approval.
Develop events and educational forums that demonstrate programs and provide an opportunity to identify potential referral sources.
Represent facility in professional forums and communicate mission in all outreach interactions.
Create outreach initiatives that will regularly communicate facility mission with referral sources through phone, email, e-blast, written notes, events, networking and other professional activities.
Experience
Required
Two or more years’ experience in sales and/or marketing required.

High School/GED or better
